The PMI mask came in two models and was manufactured by Tom Kaye of Airgun Designs.

These were Tom's first production items and were contracted by David Freeman and Jeff Perlmutter of PMI. These were vacuum formed plastic. Looks at Tom Kaye's website for more information on this mask:
